<p>The whitepaper argues that, “the possibility of preventing the development of influenza virus outbreaks and pandemics is being considered for the first time because quantitative changes in the genome have been shown to predict strain-specific outbreaks and the particular geographic locations where outbreaks will occur.”</p>
<p>Dr. Bogoch’s discovery of Replikins, their concentration, conservation, and trans-strain sharing in the influenza genomes, and their relationship to rapid replication and advance warning of virus outbreaks, permitted the prediction in 2008 of the last H1N1 Pandemic of 2009, six other correct predictions of influenza outbreaks, and of the geographic location of these outbreaks, and a prediction one year in advance of the current Foot and Mouth Disease outbreaks in Asia, the middle East and Europe. </p>

<p>In the paper, the authors suggest that “protective immunity against influenza virus infection is mediated by neutralizing antibodies, but the precise role of T cells in human influenza immunity is uncertain.” </p>
<p>Because of this, they undertook an influenza infection study, where they mapped T cell responses to influenza before and during infection. They found that “preexisting CD4+, but not CD8+, T cells responding to influenza internal proteins were associated with lower virus shedding and less severe illness,” and “these CD4+ cells also responded to pandemic H1N1 […] peptides and showed evidence of cytotoxic activity.”</p>
<p>The authors go on to suggest that “these cells are an important statistical correlate of homotypic and heterotypic response and may limit severity of influenza infection by new strains in the absence of specific antibody responses. [These] results provide information that may aid the design of future vaccines against emerging influenza strains.”</p>

<p>Annie kindly provided these whitepapers, entitled <strong>‘Coupling sensitive <em>in vitro</em> and <em>in silico</em> techniques to assess cross-reactive CD4+ T cells against the swine-origin H1N1 influenza virus,’</strong> and <strong>‘Time for T? Thoughts about the 2009 novel H1N1 influenza outbreak and the role of T cell epitopes in the next generation of influenza vaccines.’</strong></p>

<p>In the first paper, the authors look at the 2009 H1N1 influenza outbreak, and the resulting efforts to produce large quantities of vaccine doses to protect against this virus strain. “Of particular concern was the apparent lack of pre-existing antibody capable of eliciting crossprotective immunity against this novel virus, which fueled fears this strain would trigger a particularly far-reaching and lethal pandemic.” However, as the pandemic was far less severe than expected, the authors suggest that “cellular immunity to cross-conserved T cell epitopes might have played a significant role in protecting against the pandemic H1N1 in the absence of cross-reactive humoral immunity.” They propose that “pre-existing CD4+ T cells can elicit crossreactive effector responses against the pandemic H1N1 virus.”</p>

<p>In the second paper, the authors look at the role of T cell epitopes in next-gen flu vaccines. They suggest that, “there might be cross-protective immunity due to previously circulating influenza virus and the novel H1N1… Was it possible that – in the absence of cross-protective antibodies &#8211; T cells were contributing enough of a protective immune response to protect some individuals against novel H1N1?”</p>

<p>Key quotes and findings from the <a href="http://www.who.int/immunization/sowvi/en/">State of the World’s Vaccines and Immunization Report</a></p>
<p>“ Immunization is one of the most powerful and cost-effective of all health interventions.  It prevents debilitating illness and disability, and saves millions of lives every year. It  is also key to achieving the Millennium Development Goals (MDGs) – commitments made by world leaders in 2000 to reduce poverty and improve human development.  The contribution of immunization is especially critical to achieving the goal to reduce  deaths among children under ﬁve years old (MDG 4).”</p>
<p><strong>Overview:</strong></p>
<p>“WHO has estimated that if all the vaccines now available against  childhood diseases were widely adopted, and if countries could raise vaccine  coverage to a global average of 90%, by 2015 an additional two million deaths  a year could be prevented among children under ﬁve years old.”</p>
<p><strong>Vaccine development:</strong></p>
<p>“Since the year  2000, the global vaccine market has almost tripled – reaching over US$ 17 billion  in global revenue by mid-2008, and making the vaccine industry one of the fastest  growing sectors of industry. Most of this expansion comes from sales in industrialized countries of newer, more costly vaccines, which account for more than half of the total value of vaccine sales worldwide.”</p>
<p><strong>Barriers:</strong></p>
<p>“A major barrier is the underlying weakness of the health system  in many developing countries. Another is the difﬁculty in delivering vaccines through  an infrastructure and logistical support system that is often overloaded. Yet another  is a lack of understanding about the importance of vaccines – especially among  the poorest populations – and a failure to actively demand access to immunization services.”</p>
<p>“One change, however, that could seriously imperil efforts to battle inequity,  preventable disease, death, and poverty, is the collapse of global ﬁnancial markets  in the last months of 2008, and the economic slowdown that has since swept over the world.”</p>
<p><strong>Costs:</strong></p>
<p>“By 2010, the average cost of immunizing a child is projected to rise to about  US$ 18.00 per live birth. Beyond 2010, scaling up vaccine coverage with new  vaccines – such as pneumococcal and rotavirus vaccines – to meet the MDGs and  the GIVS goals is likely to raise the cost above US$ 30.00 per live birth.”</p>
